This page describes information submitted through a benefits inquiry and, when requested, a Broker of Record authorization.
No Protected Health Information
No protected health information should be submitted through the website. Do not submit medical history, diagnoses, prescription information, claims details, Social Security numbers, or other sensitive health or personal information through the contact form or any other field on this site.
Information We Collect and How We Use It
The contact form asks for your name, email, phone, role, practice name, state, benefit interest, eligible employee count when provided, and any message you choose to enter. We also record the source page and campaign information if present. We use these details to respond to your inquiry and coordinate a benefits review. Inquiry details may be emailed to the program contact.
If you request a Broker of Record authorization, the form also collects the practice address, employee count, signer title, typed signature, and your electronic authorization confirmations. The authorization process records the submission time and technical audit details, including IP address and browser information. These details are used to process and document the authorization.
The generated authorization letter is stored privately. We attempt to email it to the program contact and the signer, and the form reports whether email was accepted for delivery. This website does not currently collect employee census names or work email addresses.
Cookies and Tracking
If site analytics is enabled in Publishing settings, Replit collects page visits and selected interactions, such as benefit comparisons, discovery button clicks, directory searches, and whether a request was saved or a Broker of Record authorization was sent. Custom events include only nonidentifying labels such as benefit categories, coverage tiers, action locations, directory result counts, and delivery outcomes. We do not send search terms, form entries, names, email addresses, signatures, or employee census details as custom analytics events.
On mobile visits to the home page, we also measure loading speed, visual stability, and responsiveness. Only those three measurements are sent to our server and added to daily totals. We do not store individual readings, page addresses, form entries, or visitor identifiers with these totals. Do Not Track and Global Privacy Control settings disable this measurement.
